The Fleming County Youth Soccer (FCYSP) Fall 2008 season will be an outdoor season beginning the week of August 4^th and lasting approximately 7 weeks. The program is open to all children in grades kindergarten through 8^th grade. The fee is $30.00 and includes the 2009 FCYSP indoor spring season. (If you are registering more than one child, the fee is $30.00 for the 1^st child, $20.00 each for each additional child.) Any parent(s) that agrees to volunteer to become a FCYSP team coach, can waive the registration fee, but still needs to register his/her child/children with the program.

Games will be played at the youth soccer field located at Fleming County High School next to the student parking lot and the soccer field located behind Flemingsburg Elementary School. Team practices if any, will be set by each team’s coach(s). Your child’s coach(s) will be contacting you soon. Game days, game times, locations, and opponents will be provided at a later date. Every child must wear shin guards at all games, and shin guard socks are recommended to be worn over the shin guards.

Parents Code of Behavior

The Fleming County Youth Soccer Program is a healthy, fun, safe, and positive behavior enforced county-wide activity. The goal of the program is to incorporate moderate physical activity into our youth’s daily routine through the participation on the most popular team sport in the world. Long-term goals include the continuation of the daily physical activity by each participant as they grow, and the continued growth of Fleming County’s Middle School and High School soccer programs.

To help the program achieve, and allow our children to succeed, please observe the following sideline rules for watching you children play and practice.

1. No abusive shouting of any kind

2. No destructive criticism, or groaning directed towards anyone

3. No hysterical shouting of instructions towards any of the participants

By signing this, you agree to engage only in positive and controlled encouragement towards all members of the Fleming County Youth Soccer Program including: all players, coaches, officials, and administrators.

Consent to Participate / Treat

My child ______________________________ has permission to participate in the Fleming County Youth Soccer program. I authorize the team coach of his/her team or any designated official, officer, or agent to obtain or administer first aide for my child. I also consent to medical, surgical or related treatment as might be required for the immediate care of my child in the event such an emergency should occur. I further agree that in case of injury, I/we the parents/guardians of this child release the Fleming County Youth Soccer Program, and the Fleming County School System of any liability. I agree that my insurance coverage will be applicable in the event of injury.

This registration form can be mailed to the address below. The deadline to register is Wednesday July 30^th. There will be an on-site registration at the FCYSP youth skills soccer camp Monday July 21^st from 5:30pm – 9:00pm.

Fleming County High School Health Physicals Scheduled

Mandatory high school health physicals for golf, cheerleading, volleyball and cross country will be Tuesday, July 22, at 5:00 p.m. at Family Medicine Associates. Forms will not be available that evening but must be picked up at Fleming County Board of Education office or FCHS and completed prior to Tuesday. Anyone not attending will be responsible for scheduling and paying the cost of their physical. Board office hours are 7:00 a.m. to 4:00 p.m.
